Fort Meade Va Hospital - Fort Meade, SD. The VA hospital, built in 1956, is one of the facilities slated to close under the VA's restructuring plan. Instead, a new hospital will be built in the city of Rapid, located about 30 kilometers away.

Leaders of the nation's largest integrated health care system say they are trying to adjust to shifts in where veterans are, what treatment they need and how modern care is delivered.

Most of the 171 hospitals at the center of the broader VA system were established to serve the post-World War II population.

They are decaying and now not always where veterans live. They were not designed to provide care for veterans or a way to deliver modern medicine.

"We opened in April of 1953, so we will soon be celebrating our 69th anniversary here," said Pete Tillman, associate director of the VA Medical Center in Durham, NC. "That's older than the average age of VA infrastructure hospitals. It's about 58 years.

Durham Hospital will close and be replaced by a new facility in the same area. Reflecting the agency's shift to providing more outpatient care, the VA will also build a new outpatient facility in Durham.

The number of registered veterans is expected to increase by 20% in the next seven years.

"This is great news for the veterans we serve," Tillman said. "Our immediate and surrounding areas will have the right amount of infrastructure for inpatient services and outpatient services."

The proposal stems from a review process that began during the Trump administration and continued after President Biden took office.

This reflects several demographic changes. The VA now serves a veteran population that is slightly smaller, younger, and includes more female veterans — something many of today's hospitals didn't have when they were built.

The number of veterans is decreasing in the northeast and increasing in the south and southwest.

Thus, some objects cannot be exactly replaced. Hospitals in cities such as Northampton, Mass., and Chillicothe, Ohio, will be closed and replaced by smaller outpatient clinics.

In South Dakota, hospitals in Hot Springs and Fort Meade will be replaced by clinics, and a new medical center will be built in Rapid City between them.

"For the foreseeable future, these proposals will have little impact," VA regional director Robert McDivitt said on a teleconference with South Dakota officials. "Any changes or potential changes proposed in these proposals will take at least several years, and in many cases more than a decade."

The VA plan says it would increase the number of veterans receiving VA inpatient medical or mental health care by more than 100,000 within an hour. An additional 400,000 people attend specialized care within an hour. The number increases with VA primary care and many other types of care.

In New York, Republican US Representative Nicole Malliotakis gathered a group of veterans in front of the hospital.

"We're saying today that the proposal to close the VA hospital in Brooklyn is dead," Malliotakis said to applause.

But other politicians are also involved in this scheme. The top Republican on the Illinois House Committee on Veterans Affairs. Mike Bost issued a statement saying he wanted to make sure the offers were "vetted."

Changes must go through a lengthy review process. The next step is a bipartisan commission that will hold hearings and field trips. Any recommendations from the commission require the approval of the president and Congress.

On Monday, the United States Veterans Administration released official recommendations for the future of veteran care in South Dakota.

The 84-page report breaks down the administration of the region, which includes parts of South Dakota, North Dakota, Wyoming, Nebraska, Minnesota, Missouri and Iowa. The full report is in two volumes and has been submitted to the Assets and Infrastructure Review Commission.

Word of the report came out in early March after the VA held a conference call with Black Hills VA health care providers. South Dakota politicians issued statements criticizing the proposals.

The AIR report recommends VA cut some services to Sioux Falls, convert the emergency department to an urgent care, close the Wagner Clinic and Fort Meade Hospital entirely, and build a new VA health care system in Rapid City.

Proposed Va Hospital Changes Promise More Access, Less Cost

The report was released as part of the MISSION Act of 2018, which requires an assessment of the current and future health care needs of US veterans.

The proposals were made in part because the VA predicts veteran enrollment in South Dakota will decline over the next 10 years, the release said.

According to the report, the VA in Sioux Falls has made four recommendations for the future of the hospital, including moving inpatient and surgical services to partner health providers and converting the emergency department into an urgent care.

"The inpatient medical and surgical demand at the Sea Falls VAMC is currently low. Community service providers have sufficient capacity to absorb the demand of veterans," the report said.

A Sioux Falls community living center will add 30 beds to meet the demand for seniors, according to a news release.

The Wagner clinic will be closed entirely and services will move to either the Yankton or Mitchell or Indian Health Services clinics, the release said.

In the Black Hills, Air's proposals would close the Fort Meade and Hot Springs VA hospitals and move services to a new medical center in Rapid City. The current Rapid City VA clinic will close and new clinics will open in Sturgis and Hot Springs.

The proposal said the Fort Meade and Hot Springs hospitals were "unsustainable" as Rapid City became the largest population center in western South Dakota.

The new Rapid City VA hospital will provide specialty care, expand primary care and provide mental health services. It is not known how much it will cost to staff the entire institution.

The VA report acknowledges the difficulty of finding staff, especially trained surgeons, for the new hospital. They recommend collaborating with public health providers to find surgeons.

Through January 2023, the VA will hold listening sessions across the country to learn how the proposed proposals would affect veterans.

The proposals will be submitted to President Joe Biden no later than January 31, 2023, after which Congress and the panel will have 45 days to approve or disapprove.
